Anti-Cathepsin G is an ELISA-based, automated, in-vitro test system for the quantitative determination of IgG antibodies against cathepsin G in human serum or plasma. It is used for the differentiation of a positive pANCA immunofluorescence result. Cathepsin G antibodies are found in cases of systemic vasculitis, inflammatory rheumatic diseases (SLE, Sjögren’s syndrome, Felty’s syndrome), autoimmune hepatitis, and chronic inflammatory bowel diseases.

Cathepsins are intracellular proteases of the lysosome. Their hydrolytic activity significantly contributes to the destruction of bone matrix. Autoantibodies against cathepsin G give a perinuclear (pANCA) or atypical (xANCA) fluorescence pattern in indirect fluorescence on ethanol-fixed granulocytes. Patient sera that display such an immunofluorescence pattern but give a negative Anti-MPO ELISA result should be tested for cathepsin G antibodies, among others.
