What is Chronic Pain? Chronic pain refers to persistent or recurring pain which lasts for a longer period such as weeks, months and even years. The factors that lead to chronic pain are surgeries, accidents, nerve damage, inflammation and other medical conditions. The patients who have been experiencing pain for the past three months or more are eligible to enrol into Chronic Pain Management program. To manage chronic pain, Humhealth provides you with a comprehensive workflow, which involves a multidisciplinary approach that takes physical, emotional and psychological aspects of the pain into consideration.

Principal Care Management is also known as PCM and it is very similar to Medicare’s Chronic Care Management (CCM) with a few key differences. Under the new PCM codes, specialists may now be reimbursed for providing their patients with care management services that are more targeted within their own particular area of specialty. This program addresses the need for classification of treatment for patients with one chronic condition.

Humhealth has a complete and user-friendly transition workflow when a patient is discharged from a Facility/Hospital and moves to the patient’s home or an assisted living facility. It has a comprehensive template for both Interactive Contact and Face-to-Face Visit components. The dashboard of TCM helps the Clinical staff to track the status at each stage right from discharge from a hospital until the completion of the TCM program for 30 days. It helps the Clinical staff to ensure all the steps are completed on-time without any delay.

The purpose of the AWV is to review the patient’s wellness and develop a personalized prevention plan.

What is Remote Therapeutic Monitoring? Remote Therapeutic Monitoring (RTM) manages patients using devices that collect non-physiological data also called as therapeutic data. As per CMS guidelines, therapeutic data include pain levels, medication adherence, and therapy adherence as recorded or reported by the patient via a medical device. Both respiratory and musculoskeletal system status can be monitored through RTM.
